Once completed, please send back to our office, along with a voided check or a letter from your bank verifying the routing number and bank account number. Any forms unsigned or submitted without a voided check/letter from the bank will be returned for resubmission.

This form must be received by Omega prior to the 1st of the month you wish the draft to start. Back dues for the missed month will not be drafted. Automatic draft will only debit the current month’s HOA dues and not any other assessments or balances owed on the account, such as a special assessment. While on automatic draft, you will not receive coupon books. We will send a letter at the end of next year, to inform you of how much we will draft from your bank account for your quarterly or monthly payment.
